Survey: Health Care Transition for Youth with Disabilities December 13th, 2016

Youth Organizing (YO!) Disabled & Proud is partnering with Family Voices of California to bring youth and young adults between the ages of 13 - 26 together for their own youth-led, youth-driven health care presentations, discussions and legislative visit preparation at the annual Family Voices Health Care Summit. Health care transition can be complicated and overwhelming for everyone, but even more so for youth and young adults with disabilities who have multiple disabilities/health care needs and are navigating the systems for the first time. That's why we are inviting you to share with us what you believe youth need to know about when it comes to health care. Please take this short health survey and pass it onto others who may be interested in sharing their feedback.
